Gathering views on Multi-Disciplinary Team pilots

Summary

We carried out phone interviews with Islington service users who have experienced integrated care through Multi-Disciplinary Team pilots. These pilots are aimed at more intensive users of services. The model seeks to bring together professionals from primary care, community services and social care to deliver 'person centred' care.
